Please utilize the instructions below to assist you in registering your child(ren) for Faith Formation.

How many chidlren do you need to register?

One Child
*please complete the form below in it's entirety, including submitting your registration fee and payment.

More Than One Child
*please complete the form below in it's entirety
*when completing the registration fees portion mark fees appropriate for all your children and then pay accordingly
*when registering subsequent children you need only fill out the areas marked required, unless they are receiving a sacrament to which we ask that you fill out the sacrament section
